NEW PATIENTS

If you are a new patient of Family & Cosmetic Dentistry, you will have to fill out a new patient information form and a medical history form. To save you time when you come into the office, we have made these forms available here on our website. Please fill out the new patient information form first, and then fill out your medical history information. When you come to the office, we will have a copy of the form that you filled out for you to look over and sign. If there are any discrepancies please notify our staff so we can make the required changes. We hope that our online forms will make your dental visit more enjoyable by allowing you to input the required information at your own leisure.
